Nesta's Agreement

    COVID

    • I understand a negative COVID test is required before my stay at Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat.

    • If I have symptoms of COVID, I will immediately notify Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreatand will reschedule my stay.

    • I understand that if I have symptoms of COVID or contracted COVID, I must immediately quarantine away from others until I receive a negative COVID test.

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat does not have COVID tests available and if testing is needed or required, I must purchase a test at my own expense.

    • I understand and have read Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat’s policies and agree to the terms.

    OUR HOMES

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat is not a medical facility.

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat does not have any medical devices, nor will they act as a medical facility.

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat cannot treat me for any medical issues.

    • I understand that I should familiarize myself with my surgeon’s post op instructions particularly pertaining to an emergency.

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat will follow my surgeon’s instructions.

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat cannot prescribe medications.

    • I will discuss all prior medications with my medical teamand get instructions on continuing prior medications with my medications for surgery.

    • It is not Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat’s responsibility to provide prescription medication instructions regarding medications I bring from home.

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat can only provide medication reminders and cannot give or handout medications.

    • I understand that while at Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat, I am considered to be under the care of my surgeon and will contact he/she in the event of a medical emergency.

    MY CARE

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat is not a nursing home and I must do my best to assist myself during the recovery process.

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat has medically trained staff members who are here to assist me in my recovery process.

    • I entrust my care to the staff members of Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat to put my health first at all times.

    • I will not rely on third party influences such as social media for my care.

    MEALS

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat will provide me with 3 healthy meals per day which were approved by a nutritionist.

    • I understand that if the meals are not to my liking or satisfaction, I am allowed to utilize a food delivery service for my meal(s).

    • I understand that it is my responsibility to notify Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat of any food allergies at the time of booking and check-in of any food allergies.

    • I understand that it is my responsibility to notify Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat of my food preferences at the time of booking.

    TRANSPORTATION

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat transportation services consists of pickup and drop-off only.

    • I understand that I will be transported by Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat’s staff either by SUV or 4-door sedan.

    • I understand the standard seating in a vehicle is sitting upright in a vehicle with my seatbelt. If I request to sit or lay in a different position, I do so at my own risk and will be responsible for any tickets which may arise from such request.

    • I understand Nesta’s DollHouse Recovery House, LLC dba Nesta’s Wellness Retreat transportation services will not interfere with my massage appointments.
